Medication brought to school needs to be given to the nurse office and will only be administered when the nurse office has received this form filled out correctly. CRICS Nurse office will not be held responsible in any way for the medication that the student takes in.  Only a parent can send this form and by sending it to the nurse office this form will be seen as signed
