Student Withdrawal Form

After completing this form, please expect to hear from several departments if they require action including the accounting office, the library, the registrar, the guidance counselor, the principal, and/or the Head of School. No forms or services can be provided for your student(s) by CRICS staff until all of these departments have given approval of your complete withdrawal process for all withdrawing students. Thank you for your quick attention to these needs.
